Health Insurance:

Many health insurance companies cover the entire cost of nutrition counseling. 
To check benefits with your insurance provider, call the member services phone number listed on the back of your insurance card. Be sure to ask the following questions:
 
1. Is there a deductible?
2. Is there a co-pay?
3. How many visits are covered and over what time period?
4. Is a referral required?
5. My diagnosis is_______, will this be covered?
